非体外循环冠脉搭桥术病人术后监护期临床路径的应用与效果

目的探讨非体外循环冠脉搭桥术病人术后监护阶段临床路径的应用及其效果。方法实验组30例在术后监护期间实施临床路径,对照组34例接受常规监护治疗,比较两组临床恢复指标和满意度。结果实验组病人术后首次下床活动时间和首次排便时间显著早于对照组;两组术后主要并发症发生率无显著差异;试验组病人术后住院时间比对照组短、住院费用低;实验组病人对“护士沟通能力评价”、“术后配合介绍”、“ICU健康教育”、“康复知识教育”4项的满意度显著高于对照组。结论临床路径应用于非体外循环冠脉搭桥术病人术后监护期间,可有效降低术后住院时间和住院费用,提高病人满意度。

Application of clinical pathway for patients in early ICU stage after the operation of Off-pump Coronary Artery Bypass Grafting

To study the application and effect of clinical pathway(CP)for patients in early ICU stage after the operation of Off-pump Coronary Artery Bypass Grafting(OPCAB).Method 30 patients in the experiment group received CP in early ICU stage and 34 patiants in the control group received monitor and care.Then the patients' recovery index and satisfaction were compared in the two groups.Results The time of first leaving bed and defecation after operation in the experiment group were earlier than that of control group.There was no significant difference in the rate of main complications in the two groups.The hospital days in ICU was significantly shorter and the medical cost was lower in the experiment group than that of control group.The patients' satisfaction with "nurses' ability of communication","postoperative introduction","ICU health education" and "education of rehabilitation knowledge" were significantly higher in experiment group than that of control group.Conclusions The application of CP for patients in early ICU stage after OPCAB could effectively reduce the hospital days and medical cost,and improve the patients' satisfaction.
