青光眼睫状体炎综合征的鉴别诊断

目的分析青光眼睫状体炎综合征（Posner-Schlossmann syndrome,PSS）合并原发性青光眼的临床特点,提出PSS与原发性开角型青光眼（primary open angle glaucoma,POAG）、PSS与PSS合并原发性青光眼的鉴别诊断要点,为临床甄别及治疗提供依据。方法回顾性分析近15年收集到的145例PSS患者及166例POAG患者的临床资料,提出PSS的诊断及与原发性青光眼鉴别诊断的策略,并将其运用于临床。结果对495例PSS做出了及时的诊断和治疗;甄别出PSS合并原发性青光眼70例（开角型49例,闭角型21例）,使这些患者得到了及时正确的治疗,避免了不可复性盲的发生;纠正了将PSS误诊为原发性青光眼76例,及时终止了不当治疗。结论将PSS的诊断及鉴别诊断要点运用于临床,将极大提高医生对该类疾病的认识及诊断水平,减少误诊率,避免不当治疗。

Differential Diagnosis of Posner-Schlossmann Syndrome

Objective To analyze the clinical features of Posner-Schlossmann syndrome(PSS) combined with the primary glaucoma,propose the differential diagnosis points among PSS,primary open-angle glaucoma(POAG),and PSS combined with primary glaucoma,and supply reference for clinical screening and treatment.Methods A retrospective analysis of clinical data of 145 PSS patients and 166 POAG patients collected in the past 15 years in our hospital was performed to propose the differential diagnosis points between PSS and primary glaucoma and applied them to clinical diagnosis and treatment.Results Timely diagnosis and treatment was acquired for 495 PSS cases.Seventy-one cases with PSS combined with primary glaucoma cases(open angle in 49 cases,angle closure in 21 cases) were identified,and treated timely and correctly,and the occurrence of irreducible blind was avoided.Seventy-six PSS cases misdiagnosed as primary glaucoma were corrected and the inappropriate treatement was terminated.Conclusion The application of PSS diagnosis and differential diagnosis points in clinic will greatly enhance the doctors’ levels of understanding and diagnosis of these diseases,decrease the misdiagnosis rate and avoid inappropriate treatment.
