机械通气患者密闭式吸痰与开放式吸痰的效果比较

目的探讨密闭式吸痰与开放式吸痰在机械通气患者中应用的效果。方法方便性抽样选取2009年6月至2010年1月间在我院实施机械通气患者60例,按疾病危重度评分采用分层随机抽样法将患者分为观察组与对照组,每组各30例。对照组给予开放式吸痰,观察组给予密闭式吸痰,检测吸痰前后患者心率及经皮氧饱和度指标的变化情况,并于治疗前、治疗后3、5、7d取患者气道分泌物做细菌学培养。结果观察组患者吸痰后心率及经皮氧饱和度指标明显优于对照组,两组比较差异有统计学意义(P<0.05);观察组治疗后5、7d痰培养细菌阳性人数明显少于对照组,两组比较差异有统计学意义(P<0.05)。结论机械通气患者密闭式吸痰的效果优于开放式吸痰,值得临床推广应用。

Comparison of Application Effects of Closed and Open Suctioning of Sputum in Patients with Mechanical Ventilation

Objective To investigate the application effects of airtight sucking sputum and open sputum suction in patients with mechanical ventilation. Methods From June to January 2009,60 patients with mechanical ventilation were randomly divided into observation group and control group using stratified random sampling method by critical disease score system. The control group was given open suctioning measures and the observation group were given airtight sucking sputum measures. Changes of heart rate（HR） and transcutaneous oxygen saturation（SpO2） indicators before and after suction were detected. Specimens obtained at airway secretions Before treatment,at 3,5,7 days after treatment were prepared for bacterio- logical study. Results The levels of HR and SpO2 in patients of the observation group after sputum suction were significantly superior to those of the control group（P〈0.05）. The bacterial culture positive rate of sputum at 5,7 days after treatment were significantly lower in the observation group than in the control group（P〈0. 05）. Conclusion chanical ventilation is better The therapeutic effect of closed endotracheal suction in patients with me than that with the open suctioning,which is worth of application.
