C反应蛋白定量与中性粒细胞比值、白细胞计数联合测定在细菌感染炎症诊断中的应用

目的探讨C反应蛋白（CRP）定量与中性粒细胞比值（N％）、白细胞计数（WBC）联合测定在细菌感染炎症诊断中的应用价值。方法采用SysemxXT-1800i自动血细胞分析仪、OlympusAU640全自动生化分析仪、Siemenswalkaway96微生物分析仪，分别对确诊的320例细菌感染炎症患者和220例非细菌感染炎症患者进行CRP定量与N％、WBC测定，并作相关统计学分析。结果CRP定量、N％、WBC单项测定对细菌感染炎症诊断的灵敏度及特异度分别为91．3％、78．1％、71．3％及69．1％、90％、91．8％；CRP定量与WBC、N％与WBC两项平行联合测定的灵敏度及特异度分别为97．5％、93．7％及63．6％、78．2％；系列联合的灵敏度及特异度分别为71．2％、68．8％及97．3％、99．1％；CRP定量与N％、WBC3项平行联合测定的灵敏度及特异度为100％、61．8％；系列联合的灵敏度及特异度为67．5％、100％。经U检验，平行联合测定的灵敏度与各单项测定灵敏度之间，系列联合测定的特异度与各单项测定特异度之间比较差异均有统计学意义（P〈0．05）。结论CRP定量、N％、WBC平行联合测定可提高灵敏度，系列联合测定可提高特异度。

Resuits CRP quota, N% ,WBC the single item determination to the bacterial infection inflammation diagnosis＇s sensitivity and the peculiarity respectively is 91.3%, 78.1%, 71.3% and 69. 1%, 90%, 91.8% ; CRP quota and WBC, N% and WBC 2 parallel union determination＇s sensitivity and the peculiarity respectively is 97.5%, 93.7% and 63.6%, 78.2% ; The series union＇s sensitivity and the peculiarity respectively is 71.2% , 68.8% and 97.3% , 99.1% ; CRP quota and N% , WBC 3 parallel union determination＇s sensitivity and the peculiarity is 100%, 61.8% ; The series union＇s sensitivity and the peculiarity is 67.5%, 100%. After the U - test, the parallel union determination＇s sensitivity with various single items determination sensitivity, the series union determination＇s peculiarity has statistics significance with between various single items determination peculiarity difference （P 〈 0. 05 ）. Conclusion CRP quota, N% and WBC, the parallel union determination may enhance the sensitivity, the series union determination may enhance the peculiarity.
