腹腔镜手术在女性不孕症诊治中的应用

目的探讨腹腔镜手术对女性不孕症治疗的应用价值。方法回顾分析行腹腔镜手术治疗的186例女性不孕症的临床资料及疗效。结果186例女性不孕症病例中，盆腔炎性疾病后遗症91例、子宫内膜异位症53例、多囊卵巢综合征26例、卵巢囊肿7例、正常盆腔2例、子宫肌瘤3例、子宫畸形l例及生殖器结核3例。盆腔炎性疾病后遗症是导致不孕的主要原因（P〈0．01）。术后1年内妊娠率：盆腔炎性疾病后遗症35．16％（32／91），子宫内膜异位症37．73％（20／53），明显高于其他病因（P〈0．05）。结论腹腔镜手术可以在明确导致不孕原因的同时加以治疗，且创伤小，对盆腔干扰少，在女性不孕症诊治中具有重要价值。

The application of laparoscopic surgery on the diagnosis and treatment of female infertility

Objective To investigate the value of laparoscopic surgery on the treatment of female infertility. Methods Total of 186 female infertility patients were enrolled in this retrospective study. Data were collected to analyze the reason of infertility and the outcome of operation. Results There are 91 sequelae of pelvic inflammatory disease,53 endome- triosis,26 polycystic ovary syndrome,7 ovarian cysts, 2 normal pelvic, 3 uterine fibroids, 1 uterine malformations and 3 geni- tal tuberculosis in these infertility patients. Sequelae of pelvic inflammatory disease is the main cause of infertility （ P 〈 0. 01 ）. The pregnancy rates after 1 year of operation are 35.16% （32/91） and 37.73% （20/53） for the patients who had sequelae of pelvic inflammatory disease and endometriosis, significantly higher than other causes （ P 〈 0. 05 ）. Conclusion With less trauma and less interference of pelvis, laparoscopic surgery can be used to find out the reason of infertility and treat the patients at the same time. It is worthy for the diagnosis and treatment of female infertility.
