减少直肠癌Miles术后会阴部切口感染的方法研究

目的探索减少直肠癌miles术后会阴部切口感染的方法。方法随机选取2007年6月-2011年6月本院直肠癌Miles手术患者120例为研究对象，然后再随机分为A、B两组，两组分别采用不同的抗生素预防方法、会阴部手术的止血方法和骶前间隙的引流方法，比较两组在会阴部切口感染率、会阴部创面出血率和感染切口愈合时间的差异。结果A组与B组比较，会阴部切口感染的发生率显著降低（P〈0．05），感染切口愈合时间显著缩短（P〈0．05），会阴部创面出血发生率无显著差异（P〉0．05）。结论在做直肠癌Miles手术时，充分的预防性使用抗生素，在会阴部手术时尽量用电刀止血，用经会阴切口旁和经右髂部骶前联合负压引流方法，这样处理能够明显减少会阴部切口感染的发生率，缩短感染切口的愈合时间。

Research on preventing infection of perineal incision after rectal cancer Miles

Objective Researched on the measures which could reduce infection of perineal incision after rectal cancer miles. Methods 120 cases of rectal cancer miles were randomly chosen from July, 2006 to July, 2007, which were divided into group A and group B. Using different preventive antibiotic, stanch bleeding method and drainage in presacral space, and comparing the differences of infection rate of perineal incision, the bleeding rate in raw surface and the heal time of the in- fection of incisional wound between the two groups. Result To compare group A with group B, the infectional rate of perineal incision was obviously decreased （ P 〈 0. 05 ）, the healing time of infection was shortened （ P 〈 0.05 ）, and the incidence of bleeding had no obvious difference （ P 〉 0. 05 ）. Conclusion Using preventive antibiotic, stanching bleeding by using electrotome, using negative pressure drainage besides perineal incision and in presacral space, could obviously decrease the inci- dence.and shorten the healing time of inflection.
