肢体软组织恶性肿瘤的诊断与治疗——附20例临床分析

本文收集我院1987～1992年间收治之肢体软组织恶性肿瘤20例加以分析讨论。其中包括滑膜肉瘤(7例),脂肪肉瘤(3例),纤维肉瘤(3例),横纹肌肉瘤(2例),平滑肌肉瘤(1例),神经纤维肉瘤(1例),恶性神经鞘膜瘤(1例),恶性纤维组织细胞瘤(1例),血管肉瘤(1例)。本文简要讨论了该病的诊断、手术治疗和预后问题。作者认为正确的诊断以病史、物理检查和病理所见为主要依据,特别强调冰冻切片检查和活组织检查对指导手术方法选择的重要性。由于局部切除复发率较高,扩大切除结合放疗或辅助化疗应是首选的方法。但对行扩大切除不能保存肢体功能者,仍以截肢为宜。

Diagnosis and treatment of soft tissue malignant tumor of limbs a clinical analysis of 20 cases(Abstract)

20 cases of soft tissues malignant tumor of limbs admitted during 1987- 1992 were analysed,including synovial sarcoma(7 cases),liposarcoma (3 cases),fibrosarcoma(3 eases),rhabdomyosarcoma(2 cases),leiomy- osarcoma(1 case),neurofibrosarcoma(1 case),malignant schwannoma (1 case),malignant fibrous histiocytoma(1 case),angiosarcoma(1 case). Diagnosis,operative treatment and prognosis were discussed briefly.The author considered an accurate diagnosis mainly depend upon history, physical examination and pathological findings.The results of frozen section and biopsy is the most important for choise of operative method. Because recurrence rate is higher with local resection,radical resection combined with radiotherapy or adjuvant chemotherapy is the best method. If the function of extremity will not be preserved with radical resection, amputation is indicated.
