| 摘 要： | 比较了急性心肌梗塞溶栓治疗（溶栓组）和常规治疗（对照组）的心室晚电位及室性心律失常情况。结果：溶栓组心室晚电位发生率１１．０％（５／４６），显著低于对照组３３．０／（１８／５５），Ｐ＜０．０５，时域参数中滤波后ＱＲＳ时间和ＧＲＳ未４０ｍｓ的方报电压及频域分析中２０～５０Ｈ＿ｚ的振幅谱和能量谱面积比，溶栓组和对照组有显著差异（Ｐ＜０．０５），随访发现溶栓组猝死率（１／１０．２５％）低于对照组（５／４６，１１．０％）。结果提示：溶栓治疗使阻塞的冠脉再通，减少梗塞边缘区缺血，增强心电稳定性和传导性，使心肌梗塞后猝死率呈降低趋势。

| Abstract: | The effects of
intravenous thrombolysis on timedomain and frequency domain of
signal-averagedelectrocardiogram (SAECG)and ventricular arrhy-thmia events were assessed in
1 01 patients with acutemyocardial infarction (AMI). 46 patients receivedintravenous
urokinase(group A) and 55 receivedconventional medical treatment(group B).The inci-dence of
ventricular late potentials of group A 11.0%(5/46) was significantly different from group
B33.0%(18/55),P<0.05 Significant differences wereseen between group A and B in terms of
filtered QRSduration,root-mean square voltage of the last40msec of the filtered QRS complex in
time domainand amplitude area ratio,power area ratio in fre-quency domain(p<0.05).The
follow-up studyshowed that the mortality of group A(2.5%,1/40)was lower than the group
B(11.0%,5/46).Thefavorable effects of thromobolytic therapy was dueto patency of the
infarct-related coronary artery andthe resultant amelioration of ischemia in the borderzone of the
infarct which led to improvement of the electrical stability and conduction . |
