基质金属蛋白酶及其组织抑制剂和CD44v6在子宫内膜癌中的表达及意义

摘    要:目的:研究基质金属蛋白酶(matrix metalloproteinase,MMP)-2、MMP-9及其组织抑制剂(tissue inhibitor of metalloproteinase,TIMP)-1、TIMP-2和细胞黏附分子CD44v6在子宫内膜癌中的表达及临床意义。方法:采用免疫组织化学方法分别检测73例子宫内膜癌组织和27例正常增生期子宫内膜组织MMP-2、MMP-9、TIMP-1、TIMP-2和CD44v6蛋白的表达情况。根据子宫内膜癌的病理分级、组织学类型、淋巴结转移和肌层浸润程度不同对内膜癌进行组内分组比较。结果:内膜癌组织中MMP-2、MMP-9、TIMP-1、TIMP-2和CD44v6的表达率分别为75.3%、83.6%、75.3%、72.6%和74.0%,均高于对照组的22.2%、28.6%、57.1%、18.5%和45.0%(P〈0.05)。其中,MMP-2、TIMP-2和CD44v6随肌层浸润程度的加深而表达增强;MMP-9蛋白在组织分级低、有肌层浸润和淋巴结转移的子宫内膜癌组织中表达增强。相关分析显示,MMP-2和TIMP-2、MMP-2和CD44v6有明显的正相关性。结论:MMP-2、MMP-9、TIMP-1、TIMP-2和CD44v6在子宫内膜癌的发生、发展、浸润和转移中起到一定的作用;联合测定这些指标可为子宫内膜癌的早期临床诊断、预后判断和治疗随访提供指导作用。
