拉咪夫定对慢性乙型肝炎肝纤维化的影响

目的　观察拉咪夫定治疗慢性乙型肝炎对肝纤维化的影响。方法　将90例慢性乙型肝炎患者随机分为治疗组(45例)和对照组(45例)。对照组采用对症支持治疗,治疗组在对照组基础上加用拉咪夫定治疗;治疗前及治疗3、6、12个月后,分别采用Taqman荧光探针技术检测HBV-DNA定量和放射免疫分析法测定透明质酸(HA)、层粘连蛋白(LN)、三型前胶原(HPC3 )等血清纤维化指标进行比较。结果　治疗前与治疗后12个月相比较,治疗组差异显著(P<0 01),而对照组差异不明显(P>0 05);治疗12个月后,治疗组与对照组的各项指标进行比较,差异十分显著(P<0 01)。结论　拉咪夫定可以通过抑制HBV-DNA的复制及恢复T细胞活性而起到抗肝纤维化作用。

The Clinical Observation of Fibrosis Index Change in Patients of Chronic Hepatistis B Treated with Lamivudine

Objective To investigate the fibrosis index change in patients of chronic hepatistis B treated with Lamivudine.Methods Ninety chronic hepatistis B patients were randomized into treated group and control group,each consisting 45cases.The treated group were given Lamivudine.HBV-DNA were measured by Taqman florescence probe hybridization and the fibrosis index such as HA,LN and HPC 3 were monitored before and 3 month,6 month,12 month after treatment.The volume of normal saline infused, the volume of blood loss and bank-blood infused were recorded.Results There were significant differences in HBV-DNA,HA,LN and HPC 3 before and after treatment for 12 month in treated group (P<0.01) ,but there were no significant differences in control group (P>0.05). After treated for12 month, there were significant differences between the two groups(P<0.01).Conclusions Lamivudine has anti-fibrosis effects in patients with chronic hepatistis B through inhibition HBV-DNA reproduction and resume the T cells activity.
