颅内动脉瘤破裂急诊介入栓塞治疗

目的 评价颅内动脉瘤破裂急诊介入栓塞治疗的临床效果.方法 采用介入性微弹簧圈栓塞术,对16例颅内动脉瘤破裂进行急诊治疗(3天内),其中1例宽颈动脉瘤采用支架辅助技术栓塞.结果 16例患者的16枚动脉瘤全部成功栓塞,其中完全致密闭塞10枚、接近完全闭塞与大部分闭塞各3枚.1例前交通动脉瘤患者出现术中动脉瘤破裂,继续栓塞后达到了基本致密填塞,14例基本康复,1例有轻度神经功能缺失症状,1例死亡.结论 介入微弹簧圈栓塞急诊治疗颅内动脉瘤破裂是安全、有效的,积极、及时的栓塞治疗可有效防止动脉瘤再破裂.

Interventional embolization for emergency treatment of ruptured intracranial aneurysm

Objective To evaluate the clinical efficacy of interventional embolization with microcoils for emergency treatment of ruptured intracranial aneurysm. Methods Sixteen patients with ruptured intracranial aneurysms were treated with interventional embolization within 3 days after onset. One wide-neck aneurysm was embolized with stent-assistant technique. Results Intraarterial embolization was successfully performed in all cases. The occlusion was complete in 10 aneurysms, nearly complete in 3 and partially in 3. Intraoperative aneurysmal rupture occurred in 1 patient with anterior communicating artery aneurysm, which was embolized immediately. Forteen patients rehabilitated without disability remained, one with mild extremity disability and another one was dead. Conclusion Emergency intraarterial ernbolization of ruptured intracranial aneurysm with microcoils is safe and effective, which can effectively protect the patients from aneurysm rupture again.
