腮腺肿瘤112例临床分析

目的：对腮腺肿瘤的临床和病理学特点进行分析，探讨诊断和治疗中的注意事项。方法：对1985～2005年间收治的112例腮腺肿瘤病例的临床和病理学特点进行回顾性分析。结果：112例腮腺肿瘤男性多于女性。年龄14～80岁。良性肿瘤84例，占75％，恶性肿瘤28例，占25％。B超诊断符合率86．4％。细针吸细胞学定性诊断符合率92．9％，组织类型鉴别较困难。24例腮腺良性肿瘤直径〈2cm者行肿瘤加瘤周正常腺体切除术，获得随访18例，1例腺淋巴瘤在其它部位复发。结论：术前定性诊断应根据肿瘤的部位、大小及患者的情况选择合适的检查方法，疑难病例手术中冰冻活体组织检查是确诊的可靠依据。小的腮腺良性肿瘤实施瘤周正常腺体切除术，不增加复发率，并能减少腮腺手术的并发症。

A cliniical analysis on 112 cases of parotid gland tumors

Objective： To analyse the clinical and pathological characteristics of parotid gland tumor, and to discuss some problem on the diagnosis and treatment of parotid tumor.Method： A retrospective analysis on 112 cases of parotid gland tumor from 1985-2005 with a confirmed pathologic diagnosis was carried out. Result： 112 cases of parotid gland tumor often occurred between 14 and 80 years old,most of them were male. 84 cases were benign （75%）,28 cases malignant （25%）. The inaccurate diagnosis of B-mode ultrasound was 86.4%. That of fine needle aspiration cytology diagnosis was 92.9%. 24 cases of parotid gland tumor less than 2 cm in diameter are following the regional excision of parotid gland （resection of the tumor with its surrounding glands）,only one cases of adenolymphoma was recurrence in other parts of the parotid gland. Conclusion：For the preoperative diagnosis of parotid tumor, appropriate checks were selected based on tumor location,size and the patient＇s condition. The intraoperative frozen needle biopsy inspection is reliable for the the diagnosis of suspicious case. The regional excision of parotid gland （resection of the tumor with its surrounding glands） was the efficient method of treating the small benign tumors of the parotid gland.
