颈动脉支架成形术治疗颈动脉狭窄患者疗效观察

目的:探讨应用颈动脉支架成形术治疗症状性颈动脉狭窄患者的可行性及安全性。方法:对49例症状性颈动脉狭窄患者予以颈动脉支架成形术治疗。结果:全组技术成功率100%,术前平均狭窄率为80.5%,术后残余狭窄率<20%。术后l周内患者症状及神经功能缺损体征有所改善。术中及术后发生心率减慢23例,血压下降15例,血管痉挛7例,发生脑栓塞、高灌注综合征各1例。随访3个月～1年,有9例复查了DSA,1例出现轻度再狭窄;未发现支架移位、缺血性脑卒中。结论:血管内支架成形术是治疗症状性颈动脉狭窄安全、有效的方法,严格掌握适应证和熟练操作可降低手术风险。

Curative observation of carotid artery stent angiolplasty for treatint the patients with carotid artery stenosis

Objective:To observe the feasibility and safety of carotid artery stent angioplasty for treating the patients with symptomatic carotid artery stenosis.Methods:49 patients who were diagnosed with symptomatic with carotid artery stenosis were performed with carotid artery stent angioplasty.Results:The successful rate of stent implantation was 100%.42 patients underwent stent angioplasty with a mean pre-procedural carotid stenosis of 80.5% and the residual stenosis was less than 20%.The clinical symptoms of all patients were disappearance,the neurological function defect was recovery in all patients during a week.There were 23 cases with heart rate slow down during or after operation,the blood pressure declined in 15 cases,and there were 7 patients with vasospasm,as well as one case with cerebral embolization,one case with syndrome of high perfusion.During following up for 3 months to 1 years,9 patients were diagnosed again by DSA,only one case was found mild stenosis,and no one case was found the shift of stent or cerebral infarction.Conclusion:Stent angioplasty is a safe and efficient method for the treatment of synaptomatic carotid artery stenosis.Lowering the risk of operation is done by handling indication seriously and operation skilfully.
