OCT4在卵巢无性细胞瘤中的表达及其与临床病理特征和预后的相关性

目的:探讨OCT4在卵巢无性细胞瘤中的表达及其临床意义。方法:采用免疫组织化学SP法检测10例正常卵巢组织、54例卵巢原始生殖细胞肿瘤（28例无性细胞瘤、10例卵黄囊瘤、6例未成熟畸胎瘤、10例成熟畸胎瘤）中OCT4表达情况，并分析其与卵巢无性细胞瘤临床病理学特征的关系及对患者预后的影响。结果:OCT4在卵巢无性细胞瘤中全部呈阳性表达，而在成熟畸胎瘤、未成熟畸胎瘤、卵黄囊瘤及正常卵巢组织中不表达，差异有统计学意义（P＜0.01）。OCT4表达与卵巢无性细胞瘤临床分期（P=0.002）、肿瘤大小（P=0.019）、腹水情况（P＜0.001）、淋巴结转移（P=0.005）明显相关，与年龄及肿瘤单双侧无相关性。OCT4表达与卵巢无性细胞瘤患者生存期呈负相关。结论:OCT4的高表达可能在卵巢无性细胞瘤的发生、发展过程中起重要作用。检测OCT4的表达水平可协助卵巢无性细胞瘤的诊断及判断预后。

Correlations between OCT4 expression and clinicopathological factors and prognosis of patients with ovarian dysgerminoma

Objective:To study OCT4 expression and clinical significance in ovarian dysgerminoma.Methods:The SP immunohistochemical method was employed to detect OCT4 expression in 54 cases of ovarian germ cell tumors(including 28 cases of dysgerminoma,10 cases of mature teratoma,6 cases of immature teratoma,10 cases of yolk sac tumor) and in 10 cases of normal ovary.The relationship between OCT4 expression and clinical pathological indicators was examined.Morever,the survival rate was calculated through the Kaplan-Meier survivorship curve.Results:All tissue samples of ovarian dysgerminoma showed OCT4 positive expression.However,It wasn,t detected in normal ovary,mature teratoma,immature teratoma and yolk sac tumor.OCT4 expression was closely related to clinical stages(P=0.002),tumor size(P=0.019),ascites(P＜0.001)and lymphatic metastasis(P=0.005),and negatively correlated to patients,survival.Conclusion:OCT4 positive expression may play an important role in ovarian dysgerminoma occurrence and development.The detection of OCT4 expression can help diagnose ovarian dysgerminoma and estimate prognosis.
