子宫内膜癌预后因素的Cox回归分析

目的:探讨影响子宫内膜癌预后的相关危险因素。方法:回顾分析110例子宫内膜癌患者临床资料，对年龄、子宫内膜家族史、手术-病理分期、病理类型、组织学分级、淋巴结转移、治疗方式等因素进行单因素和多因素分析。采用 Kaplan-Meier法计算生存率，Log-rank检验进行单因素分析及各组生存率曲线分布比较，Cox回归模型进行多因素分析。结果:110例子宫内膜癌患者的生存时间为2-133个月,中位生存时间52.4个月,因子宫内膜癌死亡者24例(21.8%)。1、3、5年总体生存率分别为95.4%、85.5%、78.2%。单因素分析表明:不同病理类型、手术-病理分期、肿瘤大小、淋巴结转移、组织学分级、治疗方式的生存率差异有统计学意义（P＜0.05）。Cox风险比例回归模型进行多因素分析结果显示病理类型、手术-病理分期、组织学分级、淋巴结转移、治疗方式是影响子宫内膜癌预后的独立因素。结论:病理类型、手术-病理分期、组织学分级、淋巴结转移、治疗方式是影响子宫内膜癌预后的独立因素。

Prognostic factors of endometrial carcinoma analyzed with Cox regression

Objective:To determine prognostic factors of endometrial carcinoma.Methods:Medical records of one hundred and ten patients with endometrial carcinoma were analyzed retrospectively.Univariate and multivariate analysis methods were used to determine the prognostic factors of endometrial carcinoma.Survival analysis was performed with Kaplan-Meier estimates.Univariate and Cox-regression model were used to evaluate the prognostic factors,including age,endometrial carcinoma family history,surgical pathological stage,pathological type,histological grade，lymphatic metastasis,treatment method.Results:The survival of 110 patients was 2-133 months,the median survival was 52.4 months.Univariate analysis showed a statistically significant among different situation of pathologic type,surgical pathological stage,histological grade,tumor size,lymph node metastasis and treatment methods.Cox multivariate analysis demonstrated that independent prognostic factors for endometrial carcinoma were pathologic type,surgical pathological stage,histological grade,tumor size,lymph node metastasis and treatment method.Conclusion:Pathologic type,surgical-pathological staging,histological grade,tumor size,lymph node metastasis and treatment method were prognostic factors of endometrial carcinoma.
