单孔胸腔镜在孤立性肺结节诊治中的临床应用

目的: 探讨单孔胸腔镜在孤立性肺结节诊疗中的临床应用价值。方法:分析西安交通大学医学院第一附属医院胸外科2014年3月至2015年5月施行胸腔镜手术治疗的48例孤立性肺结节患者，其中单孔胸腔镜手术（uniportal video assisted thoracoscopic surgery，uniportal VATS）组21例，传统胸腔镜手术（conventional VATS）组27例。对两组患者的手术时间、术中出血量、术后疼痛程度进行比较。结果:单孔胸腔镜组比传统胸腔镜组手术时间短（P<0.05），术后24h、72h疼痛程度减低（P<0.05），术中出血及术后1周疼痛程度差异无统计学意义（P>0.05）。结论:单孔胸腔镜手术治疗孤立性肺结节安全、可靠，能够有效减轻患者术后疼痛，值得推广。

Clinical application of uniportal video assisted thoracoscopic surgery in the treatment of solitary pulmonary nodules

Objective:To evaluate the clinical effect of minimally invasive technique with uniportal video assisted thoracoscopic surgery (VAST) for solitary pulmonary nodule (SPN).Methods:From March 2014 to May 2015，48 patients with a solitary pulmonary nodule who had undergone operation with VAST were studied.Among them 27 patients underwent conventional VAST (three ports) and 21 patients underwent uniportal VAST.The operation time，blood loss and postoperative pain scores in the two groups were compared.Results:No serious complications occurred in two groups.The characteristics of patients（age，gender and SPN diameter） between the two groups were not significantly different (P>0.05).The operation time in uniport VAST group was significantly shorter than that of conventional VAST group (P<0.05)，the postoperation pain scores in 24h and 72h of uniport VAST group was significantly lower than that of conventional VAST group.However，there were no significantly differences of blood loss and postoperation pain scores in one week between the two groups (P<0.05).Conclusion:Compared with conventional VAST，uniportal VAST is advantageous in terms of operation time and postoperative pain as a safe，reliable and minimally invasive approach.
