危重早产儿转运与结局临床分析

目的探讨危重新生儿转运系统(NETS)对危重早产儿转运过程的影响及预后的相关性,提高早产儿的生存质量,降低早产儿的病死率和伤残率。方法对转运的早产儿461例临床资料进行分析。结果不同等级医院转诊早产儿预后不同,市级医院转诊早产儿预后好,放弃治疗及病死率为0.55%,乡镇卫生院转诊早产儿预后差,放弃治疗及病死率为20.69%,转运时间早的预后好,≤24h放弃治疗及治疗及病死率为1.12%,转运时间晚的预后差,>24h放弃治疗及病死率为9.37%(P<0.01)。结论院前急救及掌握好转运指征及转运时间是救治早产儿成功的关键,对降低早产儿病死率有着重要意义。

Transport of the critical prematures and its outcome analysis

Objective To study the effects of neonatal emergency transport system(NETS) on the transporting process for the critical premature and on the prognosis so as to raise the life quality of the prematures and to reduce their mortality and disability rates.Methods A clinical analysis of 461 cases of critical prematures transported from local hospitals to our hospital was conducted.Results The outcomes differed among 461 cases of prematures transferred from hospitals of different location(the medical levels were different).The prematures transported from urban hospitals carried a good prognosis which the abandoning treatment and mortality rate was 0.55% while those from rural hospitals carried a poor prognosis which the abandoning treatment and mortality rate was 20.69%.The earlier the transport time was,the better the prognosis was.Less than 24 hours of the transport time resulted in a 1.12% of abandoning and mortality rate.While the later the transport time was,the worse the prognosis was,more than 24 hours of the transport time resulted in a 9.37% of abandoning and mortality rate.There was statistically significant difference(P<0.01).Conclusion Pre-hospital emergency care and mastering transport indication and transport time are the key to successfully saving the lives of prematures.,and it's also remarkably significant to reduce mortality rate of prematures.
