Resurfacing the Lower Face Scar with Pre-expanded Flap of the Neck

Objective：To investigate the effect of using pre-expanded flap in the reparation of lower face scar.Method：Tissue expanders were implanted in the neck and inflated for 3 to 5 months.Then the expanded neck flap was advanced to the cephalic side to resurface the scar of lower face including the upper lip.A hole was made in the flap corresponding to the position of the mouse.Result：From January 2004 to January 2009,we used pre-expanded flap of the whole neck to resurface the post-burn scar of lower face in 8 patients.Flaps survived well in 6 patients.The majority of flap survived with only small necrosis in the distal edge in 2 patients.The wound healed with dress changing.All patients got satisfactory outcome.The period of follow-up was 13 months in average,The flaps matched well to the surrounding skin in terms of color,texture and thickness.No contracture of the flap occurred,and the mental cervical angle had good figure.The ectropion of the lower lip and drooling were rectified.Conclusion：In the patients with lower face scar,and with sufficient unharmed neck flap.The expanded flap of the whole neck is suitable to be used to resurface the burn scar in the lower face.
