慢性心力衰竭与几丁质酶-3样蛋白-1的相关性探讨

摘    要:目的:检测慢性心力衰竭(CHF)患者血浆几丁质酶-3样蛋白-1(YKL40)的水平。方法:选择2009年1月至2010年12月在广州市番禺区何贤纪念医院就诊的CHF97例作为研究组。其中NYHAⅠ/Ⅱ级组69例、NYHAⅢ/Ⅳ级组28例:选择同期40例健康体检者作为对照组。检测患者血浆YKL—40、糖化血红蛋白(HBAIc)、N-末端脑利钠肽前体(NT—proBNP)及LVEF等指标。结果:除LVEF及NT—proBNP外,研究组和对照组患者的一般资料差异无统计学意义(P〉0.05);研究组患者YKL珈水平为(108±7.5)ng/mL,高于对照组的(62±5.6)ng/mL(P〈0.05);NYHAⅢ/Ⅳ级组YKL-40水平为(118±8.2)ng/mL,高于NYHAⅠ/Ⅱ级组的(89±6.3).g/mL(P〈0.05)。结论:CHF患者血浆YKL铷水平明显升高,这可能与CHF患者存在炎症反应有关:随着心力衰竭的加重,YKL-40水平升高更为明显。

Abstract:Objective: To investigate the level of chitinase-3-like-1 protein (YKL-40) in patients with CHF. Methods: Recruitment ran from January 2009 to December 2010. A total of 97 patients with CHF, consisting of 69 cases with NYHA class I- II and 28 with NYHA class III-IV, as well as 40 age-matched individuals for health check-up were included from Hexian Memorial Hospital of Panyu District. The level of YKL-40, glycosylated hemoglobin (HBAlc), N-terminal natriuretic peptide precursor (NT-pro BNP), left ventricular ejection fraction (LVEF) were examined. Results: There was no significant difference in baseline indices between both groups ( P 〉 0.05 ) with exception of LVEF and NT-pro BNP. Patients with CHF were featured by higher level ( P 〈 0. 05 ) of YKL-40 [ ( 108.0 ± 7.5 ) ng/mL] when compared with healthy controls [ (62.0 ± 5.6 ) ng/mL ]. An elevated level of YKL-40 ( P 〈 0.05 ) was noted in NYHA class III-IV group [ (118.0± 8.2) ng/mL], but not in NYHA class I- II group [ (89.0±6.3) ng/mL]. Conclusion: The elevated level of YKL-40 in patients with CHF, which aggravates as the severity of CHF advances, could have stemmed from the inflammatory responses.
