摘    要:目的 探讨Glisson蒂横断式肝切除在解剖性肝切除中的应用。方法 2009年6月至2012年6月间我院同一手术组使用Glisson蒂横断式肝切除方法切除肝细胞性肝癌(HCC)45例,回顾性分析其围手术期的相关临床资料,并进行1年的随访。结果 45例患者中行解剖性肝切除占82.2%(37/45),在行三个主要肝蒂套带过程中发生胆漏、出血共6例,均以局部压迫或妥善缝合处理;术后1年肿瘤复发率15.6%(7/45);术后1年生存率为80.0%(36/45)。结论 Glisson蒂横断式肝切除体现区域性肝门阻断特点,避免肝门部脉管分别解剖的繁琐,符合肿瘤根治的原则,具有精准肝切除的优点。

Abstract:Objective To evaluate the application of Glisson pedicle blood occlusion in the process of precision liver resection. Methods From Jun. 2009 to Jun. 2012, 45 cases in our hospital received Glisson pedicle transaction method, and the perioperative clinical data and 1-year outcomes were surveyed retrospectively. Results The proportion of precision liver resection was 82.2% (37/45). Six cases suffered from bile leakage or hemorrhageg in the process of hanging the three Glisson pedicles, and all of them were brought to a satisfactory settlement. 1-year tumor recurrence was 15.6% (7/45), and 1-year survival rate was 80.0% (36/45). Conclusion Glisson pedicle blood occlusion not only can protect the remanent liver functions, but also have the advantages of preventing tumour spread and embodying the concept of precision liver resection.
