原发性小肠肿瘤的X线诊断

目的：加深Ｘ线对原发性小肠肿瘤诊断价值的认识。方法：回顾性分析了经钡造影发现的３２例病人的Ｘ线、手术及组织病理学表现并复习了有关文献。结果：３２例患者中原发性腺癌１６例。腺癌的主要Ｘ线表现为：（１）肠腔狭窄（２）充盈缺损（３）肠梗阻或肠套叠（４）肿瘤近侧肠管动力性扩张及逆蠕动。恶性淋巴瘤３例。主要Ｘ线表现为（１）动脉瘤样扩张（２）假性憩室样大空腔（３）较广泛的、结节状或不规则无蒂息肉样充盈缺损。良性肿瘤１３例，主要Ｘ线表现是单发或多发充盈缺损，表面光滑，有蒂或无蒂。结论：原发性小肠肿瘤的Ｘ线表现对提示原发性小肠肿瘤的特异性诊断具有典型特征。

A RADIOLOGICAL DIAGNOSIS OF THE PRIMARY SMALL BOWEL TUMORS

Objective:To deepen understanding the diagnostic value of radiological for the primary small bowel tumor Methods:Radiographic,surgical,and histopathologic findings were retrospectively reviewed in 32 patients with the barium examination and review of literature Results:The primary adenocarcinoma were demostrated in 16 patients The main x ray manifestations of the primary adenocarcinoma were:(1)stenosis(2)filling defect(3)obstruction or intussusception(4)dynamic dilanation and revered peristalsis of the bowel segment proximal to the tumor Malignant lymphoma were detected in 3 patients The main x ray findings were:(1)aneursmal(2)psudodiverticular large cavity(3)diffuse,nodular or irregular sessile polypoid filling defect In 13 patients with benign tumor,major radiographic appearance wer single or multiple filling defects with smooth surface and pedunculated or sessile Conclusion:The radiographic appearance of the primary small bowel tumor is characteristic for predicting a specific diagnosis of a primary small bowel tumor
