摘    要:目的:用CT仿真内窥镜研究解剖因素对低位直肠癌保肛的影响。方法:对我院2008年8月-2011年3月,收治的57例低位直肠癌患者术前行直肠CT仿真内窥镜检查,分析和评估骨盆径线、身高体重指数(BMI)以及肛缘距离对低位直肠癌保肛的影响。结果:收治的57例直肠癌患者行Mile手术28例,Daxion手术29例,低位直肠癌保肛成功与否和骨盆径线无显著相关(P>0.05),和BMI有相关性(P<0.05),但是狭小骨盆延长手术时间,增大手术出血量。直肠CTVE能够很好的判断肿瘤距离肛缘的距离,要优于结肠镜结果,明显减少了判断的误差(P<0.05)。结论:术前行直肠CT仿真内窥镜检查可以很好的评价骨盆因素在保肛手术中的影响,对设计手术体位,矫正骨盆曲度,模拟手术过程有很好的作用。

Abstract:Objective:To anatomical study using CT virtual endoscopy factors on the impact of rectal cancer.Methods:Months of hospital 2008.8-2011.3 months,treated 57 cases of low rectal cancer patients,preoperative CT virtual endoscopy,analysis and evaluation of the pelvic diameters,height and body mass index(BMI) and the anus distance of low rectal cancer anal impact.Results: Treated 57 cases of rectal cancer patients undergoing surgery,28 cases of mile,Daxion surgery in 29 cases,the success of rectal cancer and pelvic diameters no significant correlation(P>0.05),and BMI were correlated(P<0.05),But the small pelvis extend the operation time,increased blood loss,Daxion group of patients operated group of men and women have more significant difference(P<0.05).Conclusions: Preoperative rectal CT virtual endoscopy can be a good evaluation of the pelvic factors in sphincter preserving surgery in the influence on the design of surgical position,correction of the curvature of the pelvis to simulate surgical procedures has a good effect.
