无创双水平气道正压通气治疗中重症支气管哮喘的临床效果观察

目的观察无创双水平气道正压通气(BiPAP)治疗中重症支气管哮喘的临床效果。方法选取2011年4月—2014年2月江西省湘雅萍矿合作医院收治的中重症支气管哮喘患者46例,随机分为对照组和观察组,各23例。对照组采用常规综合治疗,观察组在常规综合治疗基础上加用无创Bi PAP治疗。比较治疗前和治疗72 h后两组患者肺功能指标〔第一秒用力呼气末容积占预计值百分比(FEV1%)和最大呼气流量占预计值百分比(PEF%)〕及血气分析指标(PaO2、PaCO2及SaO2)。结果两组治疗前FEV1%和PEF%比较,差异无统计学意义(P0.05);治疗后观察组FEV1%和PEF%高于对照组(P0.05)。两组治疗前PaO2、PaCO2及SaO2比较,差异无统计学意义(P0.05);观察组治疗后PaO2和SaO2高于对照组,PaCO2低于对照组(P0.05)。结论无创Bi PAP治疗中重症支气管哮喘效果确切,可有效改善患者肺通气功能,迅速纠正低氧及CO2潴留情况。

Clinical Effect of Non-invasive Bi-level Positive Airway Pressure on Medium to Severe Bronchial Asthma

Objective To observe the clinical effect of non - invasive bi - level positive airway pressure（BiPAP）on medium to severe bronchial asthma. Methods A total of 48 patients with medium to severe bronchial asthma were selected in Xiangya Pingkuang Cooperation Hospital of Jiangxi Province from April 2011 to February 2014,and they were randomly divided into control group and observation group,each of 23 cases. Both groups given routine comprehensive treatment,while observation group given extra non - invasive BiPAP. Lung function index including FEV1% and PEF% ,blood gas analysis index including PaO2 ,PaCO2 ,SaO2 were compared between the two groups before treatment and 72 hours after treatment. Results There was no significant differences of FEV1% ,PEF% ,PaO2 ,PaCO2 ,SaO2 between the two groups before treatment（ P 〉 0. 05）;FEV1% ,PEF% ,PaO2 ,SaO2 of observation group were higher than those of control group 72 hours after treatment,while PaCO2 was lower. Conclusion Non - invasive BiPAP has certain clinical effect on medium to severe bronchial asthma,can ef- fectively improve the pulmonary ventilation function,quickly correct hypoxia and CO2
