20例开胸术后急性呼衰的成因探究及护理对策

目的探讨开胸术后发生急性呼吸衰竭的原因及护理措施。方法对20例开胸手术后并发急性呼吸衰竭患者,通过及时综合治疗,加强对呼吸功能的护理,密切观察其临床表现,采取正确卧位、翻身、拍背、吸痰、雾化;保持有效呼吸和吸氧、维持引流通畅等护理措施的处理。结果15例患者动脉血气分析恢复正常,5例血氧饱和度在80%～85%。呼吸道感染、手术创伤、伤口疼痛刺激、术前心肺功能异常等是引起开胸术后呼吸衰竭的主要原因。结论术前准确的心肺功能的评估,术后观察及有效的护理措施是预防及治疗开胸术后急性呼吸衰竭的有效措施。

The Causes and Nursing Measures of Acute Respiratory Failure after Thoracotomy

Objective To discuss the causes and nursing measures of acute respiratory failure after thoracotomy.Methods 20 patients with acute respiratory failure post-thoracotomy.Through timely and comprehensive treatment,enhance the care of respiratory function and observe the clinical manifestations closely patency.nursing measures were provided,such as the correct decubitus,turn,shoot back,suction and aerosol;keep respiratory,oxygen effectively and maintenance of drainage.Results 15 patients with arterial blood gas analysis return normal and 5 patients got 80%-85%.Respiratory tract infections,surgical trauma,wound pain,abnormal pre-operative cardio-pulmonary function after thoracotomy,such as is caused by the main reasons for respiratory failure.Conclusion The accurate preoperative assessment of cardio-pulmonary function,postoperative care,observation and effective nursing measures are effective measures of prevention and treatment of acute respiratory failure after thoracotomy.
