2型糖尿病并发冠心病相关因素的分析

目的 :分析 2型糖尿病 (DM)患者易并发冠心病 (CHD)的高危因素。方法 :将已确诊为 2型DM的 6 8例患者 ,根据其是否并发CHD分为 2组 ,其中单纯DM 38例 ,DM并发CHD 30例。 2组病例均了解性别、年龄、体重、病程及吸烟状况 ,计算体重指数 (BMI) ,并检测血压 (SBp ,DBp)、血糖 (FBG ,PBG)、胰岛素 (FINS ,PINS)、糖化血红蛋白 (GHbA1c)、总胆固醇 (TC)、甘油三酯 (TG)、低密度脂蛋白胆固醇 (LDL c)、高密度脂蛋白胆固醇 (HDL c)、胰岛素抵抗指数 (HOMA IR)和 2 4h尿白蛋白排泄率 (UAER)。结果 :① 2组患者年龄、性别、病程及BMI、TC比较 ,无明显差异 (P >0 .0 5 )。②吸烟率、Bp、BG、INS、GHbA1c、TG、LDL c、HOMA IR及UAER ,DM并发CHD组明显高于单纯DM组 (P <0 .0 5或P <0 .0 1)。③HDL cDM并发CHD组明显低于单纯DM组 (P <0 .0 1)。结论 :Bp、BG、INS、TG、LDL C及HOMA IR及 2 4hUAER异常增高是 2型DM并发CHD的高危因素 ,应予以早期防治。

Analysis of Related Factors on Type 2 Diabetes Mellitus with Coronary Heart Disease

Objective: To analyse the high risk factors of type 2 diabetes mellitus(DM)with coronary heart disease(CHD). Methods: 68 patients of type 2 DM were divided into two groups: 38 patients of simple type 2 DM and 30 patients of type 2 DM with coronary heart disease. Smoking rate,blood pressure,blood glucose,insulin,GHbA 1c ,TC,TG,HDL c,LDL c,HOMA IR and 24 h urine albumin excretion rate(UAER)were detected in all cases,and involved in following comparison study. Results: Smoking rate,FBG,PBG,FINS,PINS,GHbA 1c,TG,LDL C,HOMA IR and UAER of type 2 DM with CHD were much higher than those of type 2 DM(P<0.05 or P<0.01). HDL C of type 2 DM with CHD were much lower than that of type 2 DM(P<0.01). Type 2 DM with CHD had no relation with TC,BMI and DM duration(P>0.05). Conclusion: Hypertention,hyperglycemia,hyperinsulinism and 24 h UAER are the high risk factors of type 2 DM with CHD and should be prevented early.
