硬膜外阻滞在带状疱疹治疗中的应用

目的　观察硬膜外阻滞与无环鸟苷治疗带状疱疹的疗效。方法　将 96例带状疱疹患者随机分为硬膜外组 (n =66 ,Ⅰ组 )和无环鸟苷组 (n =30 ,Ⅱ组 ) ,比较两组在治疗 1周前后的疼痛VAS评分 ,止痛、止疱、结痂时间及有无后遗神经痛和不良反应。结果　在治疗 1周后患者疼痛VAS评分 ,Ⅰ组较治疗前明显降低 (P <0 .0 5) ,Ⅰ组明显低于Ⅱ组 (P <0 .0 5) ;Ⅰ组的止痛、止疱、结痂时间明显优于Ⅱ组 (P <0 .0 1 ,P <0 .0 5 ,P <0 .0 5)。随访 1周时的痊愈率、总有效率Ⅰ组显著高于Ⅱ组 (P <0 .0 1 )。Ⅰ组无后遗神经痛 ,Ⅱ组后遗神经痛者达 1 3 .3 %(P <0 .0 1 )。结论　硬膜外阻滞治疗带状疱疹的疗效明显优于无环鸟苷

Application of epidural anesthesia on herpes zoster

Objective It is to observe the effect of epidural anesthesia and Acyclovir injection on herpes zoster (HZ). Methods 96 patients with HZ were randomly divided into group Ⅰ and Ⅱ. 66 patients in group Ⅰ were treated with epidural anesthesia, while 30 patients in group Ⅱ were treated with Acyclovir. Visual analogue score (VAS), pain relieving time, herpes stopping time, scab forming time, post herpetic neuralgia and side effect of two groups were compared one week before and after treatment.Results The pain VAS score of group Ⅰ one week after treatment was significantly lower than that before treatment ( P <0.05) and that of group Ⅱ one week after treatment ( P <0.05). The pain relieving time, herpes stopping time and scab forming time in group Ⅰ were significantly shorter than that in group Ⅱ ( P <0.01, P <0.05, P <0.05). One week follow up indicated that the recovery rate, total effective rate of group Ⅱ were significantly higher than those of group Ⅱ ( P <0.01). The post herpetic neuralgia rate in group Ⅱ (13.3%) was significantly higher than that of group Ⅰ (0).Conclusion Epidural anesthesia is more effective than Acyclovir on HZ.
