自制冰敷瓶局部冰敷结合米袋压迫膝关节对膝关节置换术后康复效果的临床观察

目的：观察膝关节置换术后应用自制冰敷瓶结合米袋压迫患膝与膝关节置换术后常规术后处理进行比较,研究有效减少膝关节置换术后出血量、减轻术后疼痛、改善膝关节功能活动的方法。方法：40例行初次人工全膝关节置换术（TKA）患者,随机分为治疗组和对照组,膝关节置换术后应用自制冰敷瓶局部冰敷结合米袋压迫者纳入为治疗组,共20例;膝关节置换术后常规术后处理为对照组,共20例;对两组治疗前后VAS评分、术后总出血量、术前和术后1月Lysholm评分进行统计学比较与分析。结果：两组术后12、24 h静止VAS评分、48、72 h运动VAS评分、术后1个月膝关节功能评分、术后2、12、24、48 h总出血量（总引流量）均有显著性差异（P〈0.05）。结论：TKA术后应用自制冰敷瓶局部冰敷结合米袋压迫膝关节能明显减少术后出血量,减轻术后疼痛,改善术后功能,是一种有效减少TKA术后出血、减轻术后疼痛、改善功能的措施,具有临床研究价值。

Clinical observation of Ppostoperative rehabilitation effect oppress the knee-joint with the homemade ice bottle and rice bag after Total Knee Arthroplasty (TKA) : a clinical observation

Objective： We To compared the effect of the treatment oppressing the knee- joint with the homemade ice bottle and rice bag after Total Knee Arthroplasty（ TKA） with regular postoperative treatment,so as to find an effective way of reducing postoperative blood loss and the pain after the operation and improve the knee function. Method： Totally 40 patients with TKA for the first time are were recruited involved infor this study. We,and randomly divided them randomly into 2 groups： the treatment group and the control group（ Nn = 20）,20 patients each. The treatment group was treated with homemade ice bottle and rice bag oppressing the knee- joint after TKA while the control group with regular postoperative treatment. We compared and analyzed the The data of both group including the VAS score,postoperative blood loss and Lysholm score both before and after a month of the operation were compared and analyzed. Result： It turned out significant differences among the postoperative stationary VAS score of 12 and 24 h,sport VAS score of 48 and 72 h,knee function score a month after the surgery,the total amount of bleeding after surgery 2,12,24,48 h（ total drainage volume）. Conclusion： So we concluded that o Oppress the knee- joint with homemade ice bottle and ice bag can significantly reduce the postoperative blood loss and the pain after operation and improve the knee function. It is an effective method with clinical research value.
