机器人辅助全膀胱切除术的研究进展

机器人辅助全膀胱切除术是近年新发展起来的微创手术方式。为总结机器人辅助全膀胱切除术的疗效,本文回顾近年来机器人辅助全膀胱切除术的文献,总结和比较了机器人辅助全膀胱切除术与开放手术在围手术期结果、早期手术并发症、肿瘤转归和盆腔淋巴结清扫术的情况,初步评估该微创手术与开放全膀胱切除术相比具有的优势和不足。文献统计发现与开放全膀胱切除术相比,机器人手术具有术中出血量少、平均住院时间短、肠道功能恢复时间快、围手术期并发症发生率低等优点,而且,机器人手术在短期肿瘤控制和盆腔淋巴结清扫术中也具有一定的优势。但是,仍需要长期随访和多中心随机对照研究对机器人辅助全膀胱手术作进一步的评价。

Recent advances in robot-assisted radical cystectomy

Robot-assisted radical cystectomy（RARC）is an emerging minimally invasive approach to open radical cystectomy.We review the recent literature for RARC series that have been reported in English until now.Operative parameters,perioperative outcomes,early complications,oncological outcomes and pelvic lymph node dissection were examined to evaluate this technique as a minimally invasive alternative to open radical cystectomy for the treatment of bladder cancer.Most studies report decreased blood loss,early return of bowel function,and shorter length of hospital stay for robot-assisted radical cystectomy.Furthermore,current data suggest potential benefits in perioperative and short-term oncologic outcomes with equivalent pelvic lymph node dissection as compared with open radical cystectomy.Long-term follow-up and multicenter prospective,randomized controlled trial comparisons with open radical cystectomy are needed as this technique continues to be evaluated.
