腹膜透析患者心脏结构和功能变化的超声特点

【目的】探讨维持性腹膜透析患者心脏结构和功能的改变并分析其形成原因。【方法】通过观察2005-2008年间96例终末期肾脏病持续性非卧床腹膜透析(continuous ambulatory peritoneal dialysis,CAPD)患者开始透析及透析3年后的心脏超声检查(UCG),测定其射血分数(EF)、每分输出量(CO)、左室舒张末期直径(LVDd)、左室收缩末期直径(LVDs)、室间隔厚度(LVST)和左室后壁厚度(LVPWT)、心指数(CI)等指标,同时选择92例无肾脏疾病、无风湿性疾病的患者作对照组。【结果】(1)CAPD组与对照组在CO、CI、LVDd、LVDs、LVST、LVPWT存在显著性差异(P<0.05)。(2)CAPD组3年前后上述指标比较差异有显著性(P<0.05)。【结论】由于众多影响因素使CAPD患者心脏更易发生结构和功能的改变,导致心血管事件的高发生率,且其改变的速度快。通过有效的干预有利于减少和延缓患者心血管事件的发生、发展。

Changes of cardiac structure and function in 96 cases of peritoneal dialysis patients

[Objective] To investigate the changes of cardiac structure and function in peritoneal dialysis patients and analyze the reasons for its formation. [ Methods ] Data at the time of starting dialysis and dialysis for 3 years were collected in 96 cases of end-stage renal disease under continuous ambulatory peritoneal diatysis from 2005-2008 including ejection fraction. [ Results] （1）In the CO, CI, LVDd, LVDs, LVST, and LVPWT were significantly different between （P 〈0.05）. （2） Parameters in CAPD group at the time of starting dialysis and dialysis for 3 years were significant different（P 〈0.05）. [ Conclusion ] As many factors in CAPD patients are more likely to change and leading to high incidence of cardiovascular events, by effective intervention helps to reduce and delay the occurrence and development of cardiovascular events.
