出血性脑静脉窦血栓13例临床分析

目的探讨出血性脑静脉窦血栓的诊断和治疗方法。方法回顾性分析2006年6月至2010年1月收治的13例出血性脑静脉窦血栓患者的临床资料。结果12例患者治疗后病情逐渐平稳，其中9例保守治疗患者动态复查CT显示血肿逐渐缩小，无一例血肿扩大；4例手术患者中3例恢复良好，1例病情重，术后并发下肢深静脉血栓，家人放弃治疗，自动出院。结论抗凝治疗是当前治疗脑静脉窦血栓的主要方法，即使在合并颅内出血的情况下也常作为治疗首选方式。但对于大范围及多灶性出血梗死，特别是已发生脑疝患者，外科手术清除血肿或去骨瓣减压非常必要。

Clinical analysis of 13 patients with hemorrhagic cerebral venous sinus thrombosis

Objective To explore the diagnosis and treatment of hemorrhagic cerebral venous sinus thrombosis. Methods Retrospective analysis of clinical data of 13 patients with cerebral ve- nous sinus thrombosis hospitalized from June 2006 to January 2010. Results Twelve patients be- came stable after treatment ; Nine patients of them were treated by conservative treatment and their re- suits of CT demonstrated that bematoma became smaller in size; Three of 4 operative patients recov- ered well, 1 case discharged due to severe illness. Conclusions Anticoagulant therapy is currently the main treatment of cerebral venous sinus thrombosis, even in the case of intracranial hemorrhage, often as a treatment of first choice. For large - scale and multi - focal hemorrhage infarction, particu- larly in patients with cerebral herniation, surgical removal of hematoma or decompressive craniectomy is necessary.
