全身骨显像在神经母细胞瘤骨转移中的价值

目的评价放射性核素全身骨显像诊断神经母细胞瘤骨转移的价值，探讨其对神经母细胞瘤骨转移诊断的独特优势，进而对疾病进行临床分期并指导治疗。方法49例经病理证实的神经母细胞瘤患者，术前或术后行全身骨显像，对其结果进行分析。结果49例神经母细胞瘤患儿，全身骨阳性显像27例，阳性率55．1％。27例骨显像阳性病例中，颅骨、肩胛骨、胸骨、脊柱、肋骨、骨盆及四肢骨受累分别为6例（22．2％）、2例（7．4％）、1例（3．7％）、10例（37％）、4例（14．8％）、8例（29．6％）及14例（51．9％）。单发病灶10例（37％），多发病灶17例（63％）。Ⅰ期5例，Ⅱ期9例，Ⅲ期8例，Ⅳ期27例。结论放射性核素全身骨显像对诊断神经母细胞瘤骨转移具有重要的价值，为临床分期的依据。

Value of bone imaging in neuroblastom with bone metastasis

Objective To evaluate the value of radionuclide bone imaging in diagnosing neuroblastom with bone metastasis, explore the distinct advantage of which in diagnosing neuroblastom with bone metastasis, offer clinical staging and therapeutic guidance to neuroblastom. Methods To analyze the results of bone imaging which were taken before or after surgery in 49 children with neuroblastom who were confirmed by pathology. Results Among 49 eases of neuroblastom, 27 eases had positive findings in bone imaging with the positive rate was 55.1%. Among 27 positive cases, the skull, scapula, sternum, spine, rib, pelvis and extremities involvement were 6（22.2%）, 2（7.4%）, 1 （3.7%）, 10 （37%）, 4（14.8% ）, 8（29.6% ）, 14（51.9% ） cases, respectively. Single focus was seen in 10 patients（37% ）, while multiple focus was seen in 17 patients（63% ）. Among this 49 patients, 5 cases had stage Ⅰ ,9 eases had stage Ⅱ,8 eases had stage Ⅲ and 27 eases had stage Ⅳ diseases. Conclusions Radionuclide bone imaging has great value in diagnosing neuroblastom with bone metastasis, which eoutd be evidence for clinical staging.
