超声多参数积分法评估慢性乙型肝炎后肝纤维化

目的评价超声多参数积分法对乙型肝炎后肝纤维化的诊断价值。方法对80例慢性乙型肝炎患者进行超声检查，重点检测肝包膜、肝实质回声、门静脉、胆囊壁厚度、脾脏大小、肝静脉频谱及门静脉系统血流量等7项指标，根据其不同表现进行评分，由此得出每例患者的超声总积分，并与穿刺活检结果相对照。结果超声积分与病理纤维化分期有良好相关性，尤其对区分≥S2期的中、重度患者准确率高。结论与单指标分析相比较，超声多参数积分法对肝纤维化定量诊断的准确率提高，尽管有一定局限性，仍可作为无创评估肝纤维化的有效手段。

Ultrasonographic evaluation of hepatitis B - related liver fibrosis by comprehensive score

Objective To evaluate the diagnostic value of uhrasonography with comprehensive score on hepatitis B- related liver fibrosis. Methods Color Doppler sonographie examinations were performed in 80 patients with chronic B -related hepatitis ,we observed 7 indexes including liver surface, hepatic parenchymal echo pattern, portal vein, gallbladder wall thickness, the size of spleen, hepatic venous flow spectrum and the flow of portal vein system. On the basis of these we got the total ultrasonic score of each patient, which was compared with the pathological stage of hepatic fibrosis. Results The total ultrasonic score correlated with the pathological stage of hepatic fibrosis well, it＇ s especially valuable to distinguish the patient who has moderate or severe fibrosis. Conclusions To compare with the single index, integral methods with 7 ultrasonic parameters can increase quantitative diagnostic accuracy of liver fibrosis. Although there are some limitations, integral methods is still a noninvasive method to evaluate hepatic pathological changes.
