显微外科手术切除大型听神经瘤

目的：34例大型听神经瘤显微外科手术治疗结果分析。方法：采用单侧枕下乙状窦后小骨窗及经内听道入路，运用显微外科技术，切除直径大于4cm肿瘤。结果：肿瘤镜下全切除28例(82％)，次全切除6例(18％)。面神经解剖保留20例(60％)，功能保留11例(32％)。术前尚存听力25例，术后听力保存3例(12％)，无手术死亡。结论：大型听神经瘤的治疗，采用枕下乙状窦后及内听道入路，运用显微外科技术切除肿瘤，是提高肿瘤切除率，保留面听神经功能的最佳方法。

Microsurgical treatment of large acoustic neuroma.

Objective: To summarize the experiences of microsurgical treatment in 34 cases with large acoustic neuroma. Methods: All cases were operated via the retrosigmoid approach through a small craniotomy by microsurgical technique. Results: The tumors were totally removed in 28 cases(82%). subtotally removed in 6 cases(18%) .The anatomical facial nerve reservation had been achieved in 20 cases(60% ) and functional reservation in 11 cases(32% ) .25 cases had residual hearing before operation. 3 cases(12%) preserved their hearing function after operation.No death was found during and after operation.Conclusion: In the treatment of large acoustic neuroma, using the microsurgical technique on the retrosigmoid approach of a small craniotomy for total removal is the best way to improve the resection rate of tumor and reserve the facial nerve function.
