| 摘 要： | 目的探讨实时组织弹性成像(RTE)对颈部淋巴结病变良恶性鉴别诊断价值。方法应用RTE对107例不明原因颈部淋巴结肿大的患者进行检查,检出128个肿大的颈部淋巴结进行活检。采用RTE4分评分法进行评价,并采用超声诊断仪自带的测量软件测出各个结节的应变率值。本研究对评分法采用Kappa分析,将结果由3位不同资历的医师进行盲审来作诊断试验一致性分析,最后采用受试者工作曲线(ROC)计算出弹性应变率值的临界值。所有淋巴结病变均获活检手术病理证实。结果 RTE诊断恶性淋巴结病变敏感性为92.9%,特异性为55.2%,准确性75.8%。结论 RTE具有简便、准确、良好的诊断一致性的特点,可用于颈部淋巴结疾病良恶性的鉴别诊断。
