创伤性寰枕关节脱位的诊断

目的:了解创伤性寰枕关节脱位的发病和诊断特点。方法:回顾3例创伤性寰枕关节脱位的病例,特别是其诊断过程和影像学资料,了解早期获得正确诊断的思路。结果:3例创伤性寰枕关节脱位的患者均未在急诊得到正确诊断,在观察咽后壁肿胀情况、动态观察X线平片、行CT尤其是三维重建CT检查后得到诊断。结论:平片观察枕颈区椎前软组织肿胀情况,对可疑病例进行CT检查特别是三维重建CT检查可以早期诊断创伤性寰枕关节脱位。

Diagnosis of traumatic occipitocervical dislocation

Objective:To explore the character and how to diagnose traumatic occipitocervical dislocation.Method:To review3cases with traumatic occipitocervical dislocation,especially the process of diagnosis and image of the patients including plain film,CT and MRI,to know which was the best way of giving an early diagnosis.Result:None of the3cases obtained correct diagnosis at the emergence room.The swelling of prever-tebral soft tissue,dynamic view of lateral cervical radiographs,CT especially with three dimensional recon-struction were of great value to obtain the correct diagnosis.Conclusion:To observe the edema of prevertebral soft tissue by plain films and CT scans with three dimensional reconstruction can diagnose at the early stage.
