肝管的应用解剖研究

目的：探讨肝管的长度，直径及其与周围结构的毗邻关系，为临床肝脏手术提供解剖学资料。方法：用丙烯腈－丁二烯－苯乙烯树脂（ABS）对10例新鲜肝脏进行血管灌注铸型，同时观测副肝管的数目，来源，注入部位和毗邻关系，用游标卡尺测量60例成人尸体肝脏的肝左，右管及肝总管的长度，外径和汇合角度，结果：与右肝管相比，左肝管粗，短，直；副肝管的出现率为15．7％（11／70），左、右肝管在长度，外径方面在差异（P＜0．01），副肝管的出现率高（15．7％），结论：在胆道外科手术时应注意副肝管与周围的胆道和血管的关系，以免损伤。

Applied Anatomical Studies on the Hepatic Ducts

Objective To study the length and diameter of the hepatic ducts as well as the relationship with the adjacent structures. Methods Measurements were made on the length, diameter and converging angle of the hepatic duct in 60 adult cadaveric livers. ABS infusion was made on 10 specimens to reveal the accessory hepatic duct. Results Compared with right hepatic duct, the left hepatic duct was much larger, shorter and vertically in position. The incidence of the presence of accessory hepatic duct was 15.7%(11/70). Significant difference was revealed in the length and diameter between the right and left hepatic ducts( P <0.01). Conclusions Attention should be paid to the presence of accessory bile ducts and the related adjacent inportant structures during the operation to avoid the injury of the cystic or hepatic arteries.
