绝经前后子宫卵巢良性肿瘤手术方式探讨

目的分析绝经前后子宫卵巢良性肿瘤的临床特点，探讨此类人群的手术范围。方法1998年1月～2003年12月在本院住院的子宫卵巢良性肿瘤患者124例，绝经者82例，未绝经者42例，对其症状、手术方式、病理特点进行总结分析。结果卵巢良性肿瘤以上皮性肿瘤发生率最高，子宫病变以子宫肌瘤最多。绝经后行全子宫十双附件切除者占17．7％，绝经前全子宫十双附件切除术占4．0％。以子宫疾病切除附件的病理分析中17．6％的卵巢正常，以卵巢疾病切除子宫的病理分析中37．1％的子宫正常。结论对绝经前后子宫或卵巢良性肿瘤患者是否同时行全子宫十双附件切除术式应采取慎重态度，术式选择对于患者术后生活质量的影响应引起广大妇产科医师的关注。

The Surgery Way of Uterus and Ovary Benign Tumour Around the Menopause

Objective The clinical characteristic of the ovary benign tumors of uterus around the menopause was analyzed. Methods The cases came from the first affiliated hospital of the Western medicine institute of river in 124 patient in hospital of the our academy from 1998 to december of 2003, 82 of menopause person, 42 of non-menopause. The clinical data were analyzed for their clinical characteristic and surgical treatment. Results The epithelial cancer was most common in ovary benign tumors while the mayoma of uterus around the menopause. The extensive excises of the whole uterus plus biovaries accounted for 17. 7% in the patients with menopause and for 4.0% in ones with pre-menopause There were 17. 6% of patients showed a normal ovary in pathologic examination for uterus diseases and 37. 1% showed a normal uterus in ovary diseases. Conclusion It should be careful considerated to decide if a extensive excises of the whole uterus plus biovaries would be used fro patients around menopause. And attention of obstertrician should be risen for selection of surgery way for this kind of patients.
