263例肺结核合并糖尿病临床分析

目的分析肺结核合并糖尿病的临床特点,探讨两病之间的相互影响。方法采用1:1病例对照研究方法,回顾分析我院住院肺结核合并糖尿病患者263例(研究组)及单纯肺结核患者260例(对照组),分析研究组与对照组在咯血、痰菌、合并空洞、合并感染的情况和住院治疗的近期疗效。结果研究组与对照组在咯血上无统计学差异,但在合并空洞、痰菌阳性和合并感染等临床表现上差异显著。两组病例短期住院抗结核治疗近期病灶吸收效果、痰菌阴转差异显著。结论肺结核病合并糖尿病患者在临床表现上要重于单纯肺结核患者,经常规抗结核治疗近期胸片病灶吸收较差,痰菌阴转率低,抗结核治疗时间应比单纯肺结核延长。

Clinical study of 263 patients、with pulmonary tuberculosis complicated with diabetes

Objective To study the clinical characteristic of the patients with pulmonary tuberculosis complicated with diabetes,and the interaction between these two diseases.Methods 263 patients with pulmonary tuberculosis complicated with diabetes(the studied group)and 260 patients with pulmonary tuberculosis alone(the control group)were studied.The clinical manifestation,the microbial infection and therapeutic effect of treatment were compared.Results:There were no significant differences in the symptom of hemoptysis between the two groups,while in the other clinical manifestation such as sputum smear positive,concurrent cavity and concurrent infection,the two groups have significant difference.Significant difference was also found in the absorption effect of focus of infection and the rate of sputum bacterium after anti-TB therapy for a short time.Conclusion Compared with patients with pulmonary tuberculosis alone,the clinical manifestation of patients with pulmonary tuberculosis complicated with diabetes was severer,the absorption effect of focus of infection was worse,and the sputum conversion rates was lower.The time for anti-TB therapy should be prolonged for the patients with pulmonary tuberculosis complicated with diabetes.
