老年院内获得性白念珠菌肺炎41例临床分析

目的探讨老年患者院内获得性白念珠菌肺炎的发病、治疗、危险因素及预防。方法回顾性分析我院2000年6月至2006年7月,6年中41例老年院内获得性白念珠菌肺炎患者的基础疾病、临床特点、治疗转归。结果老年院内获得性白念珠菌肺炎基础疾病以慢性阻塞性肺疾病、恶性肿瘤为主,发病前所有患者均使用抗生素,其中2种以上34例(82.93%),大部分患者长期卧床(68.29%),死亡率26.83%。结论老年患者院内获得性白念珠菌肺炎死亡率较高,合并多种基础疾病、营养状况差、长时间使用多种广谱抗生素是其主要危险因素,积极消除易感因素、早期诊治、合理使用抗生素对其有重要意义。

A study on 41 senile patients with hospital acquired nosocomical candida albicans pneumonia

Objective To explore treatment effects, risk factors and preventive measures of senile hospital acquired nosocomical candida albicans pneumonia in a senior-home hospital.Methods We retrospectively analyzed underlying diseases, clinical characteristics and treatment results. Results The underlying diseases mainly included chronic obstructive pulmonary disease (COPD) and malignant tumor. All the cases had used antibiotic, 34 cases having more than 2 (82.9%). 28 cases had to stay in bed for a long time (68.29%). The mortality rate was 26.83%. Conclusion The risk factors of senile-hospital acquired nosocomical candida albicans pneumonia included high mortality rate, concomitant multiple diseases, malnutrition, and longer use multiple broad-spectrum antibiotics. It is of significance to be aware of susceptible factors, to make early diagnose and treatment, and to be rational in the use of antibiotic medication.
