胸骨下段切口与右腋下斜切口治疗先天性心脏病的对比分析

目的探讨胸骨下段切口与右腋下斜切口治疗先天性心脏病的手术治疗特点。方法随机选取80例先天性心脏病患者,采用胸骨下段切口34例,右腋下斜切口46例进行先天性心脏病矫治手术,对术后各项指标进行观察、分析。结果腋下切口组体外循环时间(CPB时间)与胸骨下段组比较无显著差异,其肺部并发症相对较多,但切口隐蔽、美观性优于胸骨下段切口。结论腋下斜切口更具有美观性和隐蔽性,而胸骨下段切口有利于意外情况的处理,安全性高、适应证广。

Comparative study on inferior segment sternotomy and right subaxillary slant incision thoracotomy in surgery of congenital heart disease

Objective To compare the clinical results of inferior segment sternotomy and right subbaxillary slant incision thoracotomy in surgery of congenital heart disease. Methods Eighty cases with congenital heart disease were operated on by two approach, including 34 inferior segment sternotomy and 46 right subaxillary slant incision thoracotomy. The results were ananlyzed retrospectively. Results The incidence of pulmonary complications after operation in group of right subaxillary slant incision was no significantly difference than that in group of inferior segment sternotomy. The CPB time in group of right subaxillary slant incision was longer than that in group of inferior segment sternotomy. The secluding and outlook of right subaxillary slant incision was better than that of inferior segment sternotomy. Conclusion Right subbaxillary slant incision thoracotomy get an excellent effect on secluding and outlook of incision, while inferior segment sternotomy is a safe approach with wide indication, which has advantageous in treatment of the accident condition.
