益肾逍遥汤治疗肝郁脾虚肾虚型甲状腺功能减退症临床研究

目的观察益肾逍遥汤治疗肝郁脾虚肾虚型甲状腺功能减退症的临床疗效。方法选取2017年1月—2018年1月期间就诊于佳木斯中医院病房和门诊中医辨证为肝郁脾虚肾虚型的甲状腺功能减退症患者100例,采用随机对照法分为治疗组和对照组,各50例。治疗组应用益肾逍遥汤口服治疗,对照组应用左甲状腺素钠片口服治疗。治疗12周后比较两组疗效及血脂、甲状腺激素、甲状腺自身抗体水平。结果两组患者治疗后分别与治疗前比较,血清游离三碘甲腺原氨酸(free triiodothyronine,FT3)、血清游离甲状腺素(free thyroxine,FT4)、高密度脂蛋白胆固醇(high-density lipoproteincholesterol,HDL-C)均明显升高,促甲状腺激素(thyroid stimulating hormone,TSH)、甲状腺球蛋白抗体(thyroglobulin antibodies,Tg Ab)、甲状腺过氧化物酶抗体(thyroperoxidase antibodies,TPOAb)、总胆固醇(cholesterol total,TC)、甘油三酯(triglyceride,TG)、低密度脂蛋白胆固醇(low-density lipoprotein cholesterol,LDL-C)均明显下降,差异均有统计学意义(P<0.05,P<0.01);治疗后两组之间比较,治疗组对各项指标(TSH、FT3、FT4、Tg Ab、TPOAb、HDL-C、TC、TG、LDL-C)的改善均优于对照组(P<0.05,P<0.01);在总有效率方面,治疗组为96.0%(48/50);对照组为88.0%(44/50),两组比较差异具有显著性(P<0.05)。结论益肾逍遥汤治疗肝郁脾虚肾虚型甲状腺功能减退症,疗效显著,无明显不良反应。

Clinical Study of Yishen Xiaoyao Decoction on Hypothyroidism of Liver Stagnation and Spleen Deficiency

Objective To observe the clinical efficacy of Yishen Xiaoyao Decoction in treating hypothyroidism of liver depression,spleen deficiency and kidney deficiency.Methods A total of 100 patients with hypothyroidism with liver depression,spleen deficiency and kidney deficiency were selected from the wards and outpatient clinics of Jiamusi Hospital of Traditional Chinese Medicine from January 2017 to January 2018.They were divided into treatment group and control group by randomized control method.50 cases each.The treatment group was treated with Yishen Xiaoyao Decoction,and the control group was treated with Levothyroxine sodium tablets.The efficacy of the two groups was compared after 12 weeks of treatment.Results After treatment,the two groups of patients were compared with those before treatment.Serum free triiodothyronine(FT3),serum free thyroxine(FT4),high-density lipoprotein cholesterol(high-density lipoproteincholesterol),HDL-C)are significantly increased,thyroid stimulating hormone(thyroid stimulating hormone,TSH),thyroglobulin antibodies(thyroglobulin antibodies,Tg Ab),thyroid peroxidase antibodies(thyroperoxidase antibodies,TPOAb),total cholesterol(cholesterol total),TC),triglyceride(TG),and low-density lipoprotein cholesterol(LDL-C)all decreased significantly,and the differences were statistically significant(P<0.05,P<0.01);treatment Comparing the latter two groups,the improvement of the treatment group on various indicators(TSH,FT3,FT4,Tg Ab,TPOAb,HDL-C,TC,TG,LDL-C)was better than that of the control group(P<0.05,P<0.01);In terms of total effective rate,the treatment group was 96.0%(48/50);the control group was 88.0%(44/50).The difference between the two groups was significant(P<0.05).Conclusion Yishen Xiaoyao Decoction is effective in treating hypothyroidism of liver depression,spleen deficiency and kidney deficiency,with no obvious adverse reactions.
