自体冷冻下颌骨复合髂骨移植的动物实验

摘    要:目的 :论证冷冻自体下颌骨复合髂骨移植的可行性。方法 :2 0只健康雄性大耳白兔实施冷冻自体下颌骨复合髂骨移植并与对侧的新鲜髂骨移植相比较。分别在术后 2 ,4 ,8,1 2周各处死 5只动物 ,获取整块下颌骨标本作大体观察、X线检查和组织学评价。结果 :复合移植组术后 2周时冷冻骨以移植骨残余吸收为主 ,髂骨表层可见新骨生成 ,深部主要为非活性骨 ;4周时冷冻骨及髂骨均发生活跃成骨 ;1 2周时冷冻骨及髂骨皮质形成较成熟的板层骨。新鲜髂骨移植组 :愈合过程与复合移植组基本相似 ,只是在早期软骨增生异常活跃。结论 :冷冻自体下颌骨复合髂骨移植骨性愈合可靠 ,证明冷冻自体病变下颌骨复合髂骨移植修复下颌骨大范围肿瘤性缺损是可行的

Abstract:Objective: To demonstrate the feasibility of composite transplantation of frozen autogenous mandible with fresh ilium. Methods: 20 adult healthy rabbits underwent composite transplantation of frozen autogenous mandible with fresh ilium which was compared with the transplantation of fresh ilium in the opposite mandible. At 2, 4, 8, 12 weeks postoperatively, 5 animals were sacrificed respectively and the mandibles were harvested en bloc for gross observation, X ray examination and histological evaluation. Results: In composite transplantation group,at 2 weeks postoperatively, frozen mandibles displayed mainly as non vital remnants which had been absorbing,outer parts of ilia generated new bone and the inner parts showed mainly as non vital remnants. At 4 weeks, both frozen bones and ilia presented active osteogenesis. At 12 weeks, the frozen mandibles and ilia had formed lamellar bone. In the fresh iliac transplantation group,the healing process was basically the same as the composite group except for the remarkably active proliferation of cartilage in early stage. Conclusion: Bony healing in the composite transplantation is confirmed, which demonstrate the feasibility of the reconstruction of extensive tumor mandibular defects with composite transplantation of frozen lesioned autogenous mandible with ilium.
