摘    要:目的探讨肝癌演进过程中的MR信号特征及其相关病理基础.方法诱导法建立大鼠肝癌模型,于不同时相行MRI检查,对病灶行病理分级,并定量分析脂肪、糖原含量.结果51个病灶T2WI均呈高信号,T1WI分别呈高(10/51)、等(9/5D)低(32/51)信号.T1WI高信号以16~18周多见,等信号20~21周多见,低信号22~25周多见.T1WI高信号组癌组织脂肪含量明显高于癌周组织及低信号癌组织(P<0.01).结论肝细胞癌演进过程中TIWI信号呈由高到低的趋势;T1WI信号与癌组织病理分级和脂肪含量有关.

Abstract:Objective To study the signal intensity on MR images and correlated pathologies in the progression of hepatocellular carcinoma (HCC) in rats. Methods HCC models were established in 40 rats by induction. MR examinations were performed on the 16th, 18th, 20th, 21st, 22nd, 23rd and 25th week, respectively. All lesions were pathologically graded according to Edmondson-Steiner method. Fat and glycogen in the lesions and noncancerous hepatic parenchyma were quantitively analysed. Results Fifty-one lesions with more than 4 cm in diameter showed hyperintense on T2WI. On T1WI, ten showed hyperintense, 9 isointense and 32 hypointense. Seven of 10 hyperintense lesions were found between the 16th week and 18th week, Six of 9 isointense ones between the 20th and 21st week, and 27 of 32 hypointense ones between the 22nd and the 25th week. More fat contents were found in the hyperintense lesions than in noncancerous hepatic parenchyma and hypointense lesions (P<0.01). Glycogen contents were observed no statistically significant differences between cancerous tissue and noncancerous parenchyma, as well as intergroup (P>0.05). Conclusion The signal intensity on T1WI tends to change from hyperintense to hypointense in the progression of HCC, and may be contributable to the degree of histologic differentiation and steatosis in the lesions.
