关节镜下治疗不同时期胫骨髁间棘骨折

目的评价关节镜治疗早期及延期胫骨髁间棘骨折的手术方法及疗效。方法关节镜下治疗33例胫骨髁间棘骨折,其中早期手术（≤2周）17例,延期手术（〉2周）16例,比较两组膝关节功能恢复程度。结果 33例均获随访,时间26~28个月。骨折临床愈合时间6~9个月。无感染、关节强直等并发症发生。X线片显示33例均达解剖复位。关节功能恢复按Lysholm标准评定：早期手术组优秀15例,满意2例;延期手术组优秀8例,满意6例,差2例;两组比较差异有统计学意义（P〈0.05）。结论关节镜下治疗胫骨髁间棘骨折是一种非常有效的方法,早期手术（≤2周）比延期手术（〉2周）膝关节功能恢复更好。

Surgery treatment for early and delay tibial intercondylar eminence fracture under arthroscopy

Objective To evaluate the efficacy and operation techniques of the surgery treatment for early and delay tibial intercondylar eminence fracture under arthroscopy.Methods 33 patients with tibial intercondylar eminence fracture were treated under arthroscopy,among them17 cases were operated in 2 weeks（≤2 weeks）,other 16 cases were treated after 2 weeks（2 weeks）,and the function recovery of knee joints were analyzed between the two groups.Results 33 cases were followed up for 26～28 months.All fractures were unitized after 6～9 postoperative months,and no infection or ankylosis occurred.The X-ray findings showed 33 cases got anatomic reduction.According to Lysholm：15 cases operated in earlier period were excellent,2 good;8 cases in the later period were excellent,6 good,and 2 poor;the difference between the two groups was significant（P0.05）.Conclusions It is a very effective method to treat the tibial intercondylar eminence fracture with the surgery treatment under arthroscopy and the function recovery of the knee joints operated in 2 weeks≤2 weeks）,which is better than after 2 weeks（2 weeks）.
