| 摘 要： | 目的探讨鼻胃镜在经皮内镜下胃造瘘术（percutaneous endoscopic gastrostomy,PEG）中的应用价值。方法回顾分析我院9例头颈部疾病或食管癌致张口困难、吞咽困难的患者,需通过鼻胃镜行胃造瘘（transnasal PEG,nPEG）。9例通过鼻胃镜的患者均采用Introducer法,食管癌狭窄引起的吞咽困难患者6例,鼻咽癌放疗术后的张口困难患者3例。观察鼻胃镜在因食管癌、鼻咽癌导致的张口困难或吞咽困难患者中的应用情况及价值;了解其操作的时间、安全性和舒适性及并发症。结果本组9例患者均成功接受了nPEG术,未发生严重的并发症,患者的营养状况得到了改善。结论 nPEG是简便、安全、有效的胃肠道营养方法,特别是能较好地解决张口困难及吞咽困难引起的营养不良,较普通胃镜辅助的胃造瘘应用范围更广泛、更适用、更安全、更舒适,避免了此类患者的营养状况恶化,同时提高了患者的生活质量。
