塞来昔布及帕瑞昔布超前镇痛在全髋关节置换术中的应用

目的探讨塞来昔布及帕瑞昔布在全髋关节置换术中的超前镇痛效果。方法将69例行全髋关节置换术的患者随机分为超前镇痛组36例和传统镇痛组33例。超前镇痛组围手术期应用塞来昔布及帕瑞昔布,传统镇痛组术前不给予任何镇痛药物,术后2组患者出现剧烈疼痛时,辅助其他类镇痛药物治疗。分别记录2组患者术后第2、4、8、12、24、48小时VAS评分、辅助其他镇痛药物的例数及不良反应。结果超前镇痛组术后各时点VAS评分小于传统镇痛组,且术后镇痛药物辅用率为5.6%,低于传统镇痛组的30.3%,差异均有统计学意义(P<0.05)。2组术后均无过敏反应、消化道出血以及心、脑血管等系统并发症发生。结论全髋关节置换围手术期应用塞来昔布及帕瑞昔布超前镇痛,无明显不良反应,临床镇痛效果良好。

The preemptive analgesic effect of celecoxib and parecoxib in the total hip joint replacement

Objective To evaluate the preemptive analgesic effect of celeeoxib and parecoxib in patients undergone the total hip joint replacement. Methods 69 patients who received the total hip joint replacement were randomly divided into preemptive analgesic group （36 cases） and traditional emptive analgesic group （33 cases）. Preemptive analgesic group were given eeleeoxib and pareeoxib in the intraoperative period. No drugs of emptive analgesia were taken in the control group before incision. The extra drug aid and side effect were recorded. The 2 groups were compared with visual analogue score （VAS） after 2hours ,4hours,8 hours,12 hours,24 hours and 48 hours of operation. Results There were marked differences in the VAS pain scores between preemptive analgesic group and traditional eruptive analgesic group. Compared with eontrol group, the patients of preemptive analgesic group had significantly lower VAS pain scores after the operation, and the usage rate of postoperative other analgesic drug was 5.6%, which was lower than 30.3% of the traditional emptive analgesic group, and the differences were statistically significant （P 〈 0.05 ）. No serious complications and anaphylaxis was found in all cases. Conclusion Pre-operation celecoxib and parecoxib administration in patients undergoing the total hip joint replacement can reduce postoperative other analgesics consumption,with no obvious side effects. The clinical effect and security are excellent.
