改良腹腔镜筋膜内子宫切除与开腹子宫次全切除疗效对比观察

目的 探讨改良腹腔镜筋膜内子宫切除术的手术方法和临床效果。方法 妇科良性疾病需行子宫切除的住院患者127例,改良腹腔镜下筋膜内子宫切除术组64例（改良CISH组）和开腹子宫次全切除术组63例（开腹组）,对比两组平均手术时间、出血量、住院时间,术后最高体温、发热率、镇痛用药率、排气时间等方面有无差异。结果 两组平均手术时间、出血量、住院时间、术后最高体温、发热率、镇痛用药率、排气时间等方面比较,差异均有显著性意义（P〈0.01）。结论 改良腹腔镜筋膜内子宫切除术具有手术时间短、出血量小、术后病率少、术后性功能恢复快等优点。

Comparison of modified intrafascial laparoscopic hysterectomy and subtotal abdominal hysterectomy

Objective To study the modified intrafascial laparoscopic hysterectomy and its clinical results. Methods From January 2006 to August 2006, 127 patients with benign gynecologic diseases requiring hysterectomy were operated upon either by modified intrafascial laparoscopic hysterectomy or subtotal abdominal hysterectomy. The average operative time, blood lose, length of hospital stay, highest postoperative body temperature, fever rate, dosage of analgesic use, and the time for bowel movement restoration were compared between the two surgical styles. Results There were significant differences in terms of average operative time, blood lose, length of hospital stay, highest postoperative body temperature, fever rate, dosage of analgesic use, the time for bowel movement restoration between the two groups (P < 0.01). Conclusion The modified intrafascial laparoscopic hysterectomy, with the advantages of shorter operative time, less blood lose, less complications and quicker restoration of sexual function after operation, is worthy of wide clinical application.
