乌鲁木齐市基层医疗卫生机构儿童保健服务现状调查

目的分析新疆乌鲁木齐市儿童保健服务工作现状,探寻相关影响因素及基层医疗卫生机构儿童保健工作中存在的主要问题。方法 2018年1—3月共调查乌鲁木齐市七区一县88家基层卫生服务机构,通过问卷及2016—2017年妇幼卫生年报数据,回顾性了解儿童保健相关科室配备、人员构成及服务能力。结果 2016—2017年乌鲁木齐基层医疗卫生机构儿童保健门诊平均设置率为84.10%;儿童保健服务人员以兼职为主(70.3%),学历以大专最多(51.3%),临床医学专业仅占 29.7%;2017 年较 2016 年 7 岁以下儿童健康管理率(χ^2=105.720)和 3 岁以下儿童系统管理率(χ^2=23.915)有所提高(P<0.05);6 个月内纯母乳喂养率(χ^2=1.983)及新生儿访视率(χ^2=1.295)并无变化(P>0.05)。结论乌鲁木齐市各区(县)基层医疗卫生机构儿童保健服务提供不均、保健门诊设置不全,已设置儿童保健门诊的人员配备不足、流动性大、资质过低,个别服务项目指标2017年较2016年变化不明显。

Current situation of child health care services in basic medical and health institutions in Urumqi

Objective To analyze the current situation of children health care in Urumqi, explore the relevant influencing factors and the main problems of children health care in basic medical and health institutions. Methods There were 88 basic health service institutions surveyed in seven districts and one county of Urumqi. The situations of setting clinic of child health, staffing and ability of service were investigated by questionnaire and review of annual report from 2016 to 2017. Results The average rate of clinic setting of child health was 84.10% in the basic medical and health institutions in Urumqi from 2016 to 2017. Part-time workers (70.3%) and junior college of education (51.3%) were the dominant among the child health service staff in the basic medical and health institutions. The persons in clinical medicine speciality accounted for 29.7% only. Compared with 2016, the health management rate of children under 7 year-old (χ^2=105.720) and system management rate of children under 3 year-old (χ^2=23.915) increased in 2017 (P<0.05). There were no statistical differencs, P>0.05 in the rate of exclusive breastfeeding (χ^2=1.983) and neonatal visit (χ^2=1.295). Conclusions The supplies of child health services are imbalanced among the basic medical and health institutions in Urumqi. The child health clinics are not set completely. The staff is insufficient, unstable and their qualification is low. There are not obvious changes in several service items in 2017 than in 2016.
