完全腹腔镜下低位直肠前切除术+预防性回肠造口术

（1）暴露乙状结肠系膜根部与后腹膜粘连形成的黄白线，由此进入Toldt's间隙，拓展间隙至肠系膜下动脉根部，完成D3淋巴结清扫；（2）在直肠固有筋膜和腹下神经前筋膜之间分离，可以完好地保留盆腔自主神经系统从而完成全直肠系膜切除术；（3）完全腹腔镜下乙状结肠-直肠端端吻合；（4）预防性回肠袢式造口，皮内浆肌层连续缝合术在完全腹腔镜下直肠癌根治术中的应用。

Totally laparoscopic low rectal anterior resection & Prophylactic ileostomy

(1)Yellow-white borderline formed by the adhesion of the root of sigmoid mesocolon and the posterior peritoneum,from which the Toldt's space was entered,is exposed to the root of inferior mesenteric artery to complete D3 lymph node dissection;(2)Dissection between the rectal fascia propria and the anterior fascia of the hypogastric plexus can achieve the preservation of the pelvic autonomic nervous system intact and completion of the Total mesorectal excision.(3)An end-to-end anastomosis of sigmoid colon and rectum was performed totally laparoscopically.(4)Prophylactic loop ileostomy using continuous suture of dermis and seromuscular layer was performed in totally laparoscopic radical resection for rectal carcinoma.
