近红外光谱肌肉组织血氧定量监测技术的研究--模型实验与蒙特卡罗仿真研究

摘    要:近红外光谱法组织血氧检测技术可实现对人体组织肌肉、脑组织含氧浓度的无创伤长时间连续监测 ,具有广阔的应用前景。但是 ,由于光在人体组织内的强散射现象和人体组织构造的复杂性、个体差异性 ,现有的监测方法还不能实现真正意义上的定量监测。本研究通过模型实验以及蒙特卡罗仿真研究定量地分析了监测肌肉组织血氧时皮肤、皮下脂肪等对测量结果的影响 ,并提出了一种可消除皮肤、皮下脂肪影响的近红外光谱肌肉组织血氧定量监测方法。经过理论分析其监测误差可控制在 10 %以内。所提演算方法还有待相关仪器开发完成后进行临床研究加以确认。

Abstract:Near-infrared spectroscopy (NIRS) has been proved to be a useful technique for long-time noninvasive monitoring of tissue oxygenation. However,because of tissue light scattering,inhomogeneity in tissue structure and discrepancy in tissue structure among subjects, quantification of the monitoring data remains an unsdved problem. In this paper, the influences of skin and the subcutaneous fat layer on the measurement of muscle oxygenation of using NIRS are examined through simulations and experiments.A new algorithm for quantifying the measurement results is proposed, which takes into account the influence of the overlying layers. The error induced by non-linearity is allowed smaller than 10 % the ratically. Further clinical experimental study is needed to estimate the proposed algorithm.
